The police have initiated a criminal case and prosecuted Phan Ho Hoang Phuc, Director of the Y Thanh Tam General Clinic, Dong Nai.
The Investigation Police Agency, Long Thanh District Police, has issued a decision to initiate a criminal case and prosecute Phan Ho Hoang Phuc (born in 1994, residing in Long Thanh District) to investigate the act of "forging documents of agencies and organizations".
According to initial verification by authorities, some businesses in Long Thanh district received inappropriate social insurance leave certificates from employees.
Afterwards, the Dong Nai Department of Health Inspectorate conducted an inspection and determined that the place that issued the leave certificate was Y Thanh Tam General Clinic.
Checking this clinic, the joint force discovered that the clinic was storing many certificates of leave from work to receive social insurance with the names of many doctors but not signed.
Dong Nai Province Department of Health has requested the clinic to stop operating and revoked its medical examination and treatment license.
